Returning Student Athletes

All student-athletes that have participated in athletics at Folsom Lake College, American River College, Cosumnes River College, or Sacramento City College within the past year are considered “returning student-athletes” and must complete the following instructions.

Please make sure to complete all steps. Many times, students forget to complete step 6 & 8.

1. Open web browser and log onto LRCC2.ATSUSERS.COM
2. Enter your personal information and click on the “Login” button:
   • Student ID: Your personal student ID number (DO NOT ENTER THE “W”)
   • Password: The password that you set when you originally created an account.
   • Database: atslrcc
     1. If you are unable to access your account please contact FLC AT Staff to reset your password.
3. Begin with the “General” tab.
   • Fill in all required fields (highlighted in yellow) and save. If everything is up-to-date, click the “Verify Athlete Information” button at the bottom of the page.
4. Click the “insurance” tab.
   • If your insurance has not changed, click “Verify Insurance Information.”
   • If your insurance information has changed, use the “Add,” “Edit,” or “Delete” buttons to update.
5. Click the “Contacts” tab.
   • If your emergency contacts have not changed, click “Verify Emergency Contact Information”.                                   
   • If your emergency contact information has changed, use the “Add,” “Edit,” or “Delete” buttons to update.
     1. 2 emergency contacts are recommended.
6. Click the “Forms” tab.
   • Click on the down arrow in the “Form Name” drop-down menu.
   • Choose the form you would like to work on.
   • Click the “new” button. This will bring up a blank form.
     1. You may need to scroll down to see the form.
   • Complete all required fields on each form including clicking the “I agree to use electronic records and signatures” box.
   • Type your name in the “Signed By” box and click the “Sign” button.
   • Click the blue “Save” button at the end of each form.
   • The following five (5) forms are required for returning student athletes.
     1. 1. Athlete Concussion Statement
        2. Athlete Accident Insurance Info
        3. Returning Athlete Questionnaire
        4. Waiver and Assumption of Risk
        5. Covid-19 History Questionnaire
7. Physical – All student athletes must complete a yearly physical and complete an LRCCD Pre-Participation Physical Examination Form (PPE Form).

Steps to complete a physical:

• Click HERE to download and print the LRCCD PPE Exam Form.
• Bring this form to your physician. (Must be an M.D. or D.O. only)
• Make sure your physician fully completes and signs the form.
• Once completed, the form must be emailed to the FLC AT Staff.
  • If you do not have access to a scanner, take a picture and attach it to an email. Make sure that all writing and signatures are clearly visible.

8. Once steps 1-8 have been completed, contact FLC AT Staff to confirm completion.
